Forms for Current Undergraduate and Postgraduate Coursework Recipients
This page provides all the forms you may require throughout the tenure of your scholarship at the University of Sydney

- Banking Authority Form - Please provide your banking details to the Scholarships Office so that future payments or prizes can be paid directly to your bank account.
Tip: Ensure that the name of your bank account and your name on your student registration (ie through the Student Centre) are identical. We cannot pay to an account with a different name.
- Show Cause Form- Details about restrictions or problems you have faced during your studies which has prevented or incumbered you from meeting the academic requirements of a scholarship.
Tip: If you have had an illness you will need to provide medical certificates or verification, as you would when seeking special consideration at the Student Centre.
Tip: If you are unsure of what constitutes sufficient proof or cause then It is better to be sure than to guess.
- Commonwealth Scholarship Suspension Form- Only for students holding a Commonwealth Equity Scholarship. These include; Commonwealth Accommodation Scholarships (CAS), Commonwealth Education Costs Scholarships (CECS), Indigenous Commonwealth Education Costs Scholarships (ICECS), Indigenous Accommodation Scholarships (ICAS), National Priority Scholarships (NPS) and National Accommodation Scholarships (NAS).
