Proceed with formal investigation including interviews and collection of documents.
Report prepared.
Are findings adverse?
If no, go to step six.
If yes, go to step seven.
Step six
Report referred to relevant delegate for any action.
Advice of outcome provided to complainant (unless anonymous).
File closed.
Step seven
Advice of outcome provided to complainant (unless anonymous).
Affected person receives report and is asked to respond.
