Step one

Allegation is made.
Is it anonymous?

If no, go to step two.
If yes, go to step three.

Step two

Investigator meets with complainant.

Allegation is assessed by Audit and Risk Management
Is there evidence for further investigation?

If no, go to step four.
If yes, go to step five.

Step four

Complainant informed of assessment outcome (anonymous complainants cannot be contacted).
File closed.

Step five

Proceed with formal investigation including interviews and collection of documents.
Report prepared.
Are findings adverse?

If no, go to step six.
If yes, go to step seven.

Step six

Report referred to relevant delegate for any action.
Advice of outcome provided to complainant (unless anonymous).
File closed.

Step seven

Advice of outcome provided to complainant (unless anonymous).
Affected person receives report and is asked to respond.
Go to step eight.

Step eight

Report and responses forwarded to relevant delegate for any action.
Advice of outcome provided to complainant (unless anonymous).
File closed.
