Forms

These forms are available so you can submit an ICT help request and provide the necessary information required by the ICT Helpdesk to attend to your request. When you submit your form, you will receive a ticket number for your request via email. Keep the ticket number as a record and use it as your reference if you need to contact the ICT Helpdesk regarding this job.

UniKey account management

Form

Request a role based account
This form requests the creation of role-specific UniKey accounts. Please ensure that a role specific email alias will not suffice before completing and sending this form. 

New UniKey Account
This form allows support staff to request the creation of new Unikey Accounts for staff members, contractors, or persons affiliated with the University. 

Note: All permanent and contract staff have UniKey accounts created automatically for them once they are registered in the HR System.

Administration systems

Form 

Reset My Citrix password, SecurID PIN, PeopleSoft or Onestop password
This form allows users of University Administration Systems (e.g. Finance, Peoplesoft HR, Flexsis) to request a reset of their Citrix password, SecureID pin, PeopleSoft or Onestop password. A Helpdesk staff member will contact you by phone once this request has been completed. 

PeopleSoft Financials Access 
Access to PeopleSoft Financials is provided to authorised personnel who have completed appropriate training. For information about training visit the University of Sydney Finance webpages

Hyperion Finance Access     Hyperion HR Access
Access to Hyperion is provided to authorised personnel who have completed appropriate training. For information about training visit the University of Sydney Finance webpages

New Records Online User
Records Online provides University Staff access to the Records Management Services database, also known as TRIM.

FlexSIS/Sydney Student Access
Note: For access to Sydney Student please complete the FlexSIS access form.
Access to FlexSIS is provided to authorised personnel. Once you have been approved you will then need to follow the three step process. For more information click here.

Fixed telephony

Form 

Disconnect an existing phone extension
This form requests the disconnection of an existing connected phone extension. 

New phone or reassignment of extension (ie. name change)
This form requests the installation of one or more new phone ports or the activation of an existing deactivated port. It can also be used to reassign an existing extension to a new staff member 

Relocate an existing phone number
This form requests the migration of an existing University extension number to another physical location. 

Phone Faults
This form allows the electronic lodgement of any Phone Faults for Landline Extensions around the University.

Other Telecoms Request
This form allows the electronic lodgement of miscellaneous telecoms work such as changes to access levels (STD) or voicemail PIN resets.  

Mobile phones

New mobile handset with a new mobile number
Staff members may request a new mobile handset with a new number using this form.  
New mobile handset and transfer of existing mobile number
This form can be used to request a new mobile handset and arrange to have an existing mobile number transferred to University ownership.  
Replacement mobile handset
Staff members may request a new mobile handset for an existing, University owned mobile number using this form.   
New mobile number for an existing handset
This form is suitable for staff members who need to request a new mobile number for an existing University owned phone. 
Transfer an existing mobile number to the University
Staff members may use this form to request that an externally owned mobile phone number be transferred to be managed by the University.

This form allows you to request a new server. Please note that you should consult your IT Manager before requesting a server in order to ensure the correct solution is designed for your needs.

Content Management System (CMS)

Note: CMS Access requests must be submitted by a webmaster.

Alter existing CMS access
Use this form to request an alteration to a users existing CMS access.  
Remove CMS access
This form is for requesting removal of access to the CMS.
