Forms

These forms are available so you can submit an ICT help request and provide the necessary information required by the ICT Helpdesk to attend to your request. When you submit your form, you will receive a ticket number for your request via email. Keep the ticket number as a record and use it as your reference if you need to contact the ICT Helpdesk regarding this job.

UniKey account management

Form

Request a role based account
Select ICT Services > UniKey and account management > UniKey- New Role-Based Account
This form requests the creation of a role-specific UniKey account. If you would like to request a role specific email alias instead, please contact the ICT Helpdesk on x16000.

New UniKey Account
Select ICT Services > UniKey and account management > UniKey- New Account
This form allows support staff to request the creation of new Unikey Accounts for staff members, contractors, or persons affiliated with the University. 

Note: All permanent and contract staff have UniKey accounts created automatically for them once they are registered in the HR System.

Administration systems

Form 

Reset My Citrix password, SecurID PIN, PeopleSoft or Onestop password
Select ICT Services > UniKey and account management > Password Reset- Citrix, SecurID, PeopleSoft, Onestop
This form allows users of University Administration Systems (e.g. Finance, Peoplesoft HR, Flexsis) to request a reset of their Citrix password, SecureID pin, PeopleSoft or Onestop password. A Helpdesk staff member will contact you by phone once this request has been completed. 

PeopleSoft Financials Access 
Select ICT Services > UniKey and account management > PeopleSoft Access
Access to PeopleSoft Financials is provided to authorised personnel who have completed appropriate training. For information about training visit the University of Sydney Finance webpages

Planning and Reporting Finance Access
Select ICT Services > UniKey and account management > Finance- Planning & Reporting Access
Planning and Reporting HR Access
Select ICT Services > UniKey and account management > HR- Planning & Reporting Access

New Records Online User
Select ICT Services > UniKey and account management > Records Online- New User
Records Online provides University Staff access to the Records Management Services database, also known as TRIM.

FlexSIS/Sydney Student Access
Note: For access to Sydney Student please complete the FlexSIS access form.
Access to FlexSIS is provided to authorised personnel. Once you have been approved you will then need to follow the three step process. For more information click here.

Fixed telephony

Form 

Disconnect an existing phone extension
Select ICT Services > Phones > Disconnect landline phone
This form requests the disconnection of an existing connected phone extension. 

New phone or reassignment of extension (ie. name change)
Select ICT Services > Phones > Get a new landline phone and extension
This form requests the installation of one or more new phone ports or the activation of an existing deactivated port. It can also be used to reassign an existing extension to a new staff member 

Relocate an existing phone number
Select ICT Services > Phones > Relocate your phone number
This form requests the migration of an existing University extension number to another physical location. 

Phone Faults
Select ICT Services > Phones > Landline phone service request
This form allows the electronic lodgement of any Phone Faults for Landline Extensions around the University.

Other Telecoms Request
Select ICT Services > Phones > General Request
This form allows the electronic lodgement of miscellaneous telecoms work such as changes to access levels (STD) or voicemail PIN resets.  

Mobile phones

Form 
New mobile handset
Select ICT Services > Phones > Order a New Android Phone OR Apple iPhone OR Nokia Phone.
Staff members may request a new mobile handset with a new number, or arrange to have an existing mobile number transferred to University ownership.
Replacement mobile handset
Select ICT Services > Phones > Order a New Android Phone OR Apple iPhone OR Nokia Phone.
Staff members may request a new mobile handset for an existing, University owned mobile number using this form.   
New mobile number for an existing handset
Select ICT Services > Phones > General Request
This form is suitable for staff members who need to request a new mobile number for an existing University owned phone or to request that an externally owned mobile phone number be transferred to be managed by the University.

Request a new server
Select ICT Services > Core technology > Request New Server
This form allows you to request a new server. Please note that you should consult your IT Manager before requesting a server in order to ensure the correct solution is designed for your needs.

Content Management System (CMS)

Note: CMS Access requests must be submitted by a webmaster.

Form 

Request, Alter or Remove CMS access
Select ICT Services > UniKey and account management > CMS User- New, Change, Delete
This form is used to request access for new CMS users, or alter/remove existing access.

Video, web and data conferencing
