Compare the career profiles of a cohort of Standardbred horses that first raced as 2-year-olds with those that started their racing careers at a later age. Method??? Retrospective analysis of the racing records of all foals born in New South Wales in the 2000 foaling season. Results??? The career records of 999 horses were analysed. Almost half (43.9%) first raced as 2-year-olds and one-third (33.9%) as 3-year-olds. The median career duration for horses that first raced as 2-year-olds was 2.93 years (interquartile range (IQR) 2.70-3.16), which was significantly greater than the median for horses that first raced at 3, 4 or ???5 years old (P < 0.001). Males, and horses that first raced as 2-year-olds, earned significantly more prize money than females or horses that started racing aged ???3 years (P < 0.001). The population median number of career starts was 28.0 (IQR 8-64). Males, and horses that first raced as 2-year-olds, had significantly more career starts than females or horses that started racing aged ???3 years (P < 0.001). Conclusion??? The following information was collected: client demographics and history; expectations, and perceptions of 12 dimensions relating to the service received; satisfaction with previous care providers; reasons for seeking therapy; causes of previous dissatisfaction; the criteria used to assess quality; and a global assessment of satisfaction. The response rate was 69.3%. Overall satisfaction was best correlated with the therapist''s willingness to discuss positive and negative aspects of treatment (r = 0.71). The findings indicate that dissatisfied clients change health care providers, as predicted by the consumer model. This calls into question the value of surveys administered to clients during therapy, as the continuation of treatment implies a degree of satisfaction. The highest expectations were recorded in the six dimensions related to the client-therapist interaction. The measurement of expectations in the domains assessed added little value as expectations were universally high. Abnormalities in sexual arousal may contribute to its development, and to the failure of pharmacological therapies. However, there are few objective ways of assessing arousal. Sexual arousal has been shown to affect vibrotactile detection thresholds (VDT) in young, healthy men. The present study assessed the effects of sexual arousal on VDT in middle-aged men with and without ED in order to determine whether differences exist between the groups and if such differences may be useful in the evaluation of ED. Methods: VDT in the right index finger of 15 heterosexual men (mean age 74.3 +/- 6.0 years) who had been formally diagnosed with ED (ED group) and 16 men (mean age 68.0 +/- 6.6 years) who reported no sexual dysfunction in the past 6 months (erectile function (EF) group) were measured before and after watching erotic and control videos using a forced-choice, staircase method at frequencies of 30, 60 and 100 Hz. A mechanical stimulator was used to produce the vibratory stimulus. Results were analysed using repeated-measures analysis of variance. Results: There was no significant effect of watching the erotic video on VDT in subjects in the ED group. In the EF group, VDT was significantly lower at 60 and 100 Hz after watching the erotic video. There was no change in VDT after watching the control video in either group. This relationship was investigated in 30 healthy heterosexual males, who viewed both erotic and nonerotic videos. A questionnaire was used to assess the extent of sexual arousal. The grip strengths of both hands were measured with a five-position (P1-P5) dynamometer, before and after watching the videos. After watching the erotic video, there was a statistically significant reduction in grip strength for the P2 position, with nonsignificant overall reductions in grip strength for all other positions tested. No such effect was observed in control tests.
