Thesis work

Thesis title: Factors influencing short and long-term outcomes after surgical resection of pancreatic cancer.

Thesis abstract:

Pancreatic cancer is the 5th most common cause of cancer-related death in Australia. Unfortunately, the majority of patients present late in their course of illness with locoregional invasion or distant metastases. Approximately 25% of patients present with localised disease that is amenable to potentially curative resection. However, more than 75% of these patients eventually develop locoregional or distant recurrence, eventually succumbing to their disease within two to three years.

Pancreatic resection is a major undertaking for most patients, associated with high rates of morbidity and mortality, leading to a delay in the commencement of much needed systemic adjuvant therapy. This is compounded with the long-term effects of exocrine pancreatic insufficiency, post-pancreatectomy diabetes mellitus, malnutrition, and disorders of gut motility. Therefore, a significant proportion of patients endure the morbid short- and long-term effects of pancreatic resection whilst deriving minimal oncological benefit. In order to optimise care of the pancreatic cancer patient, there is a need to be able to identify predictors of both short and long-term outcomes after resection. Specifically, this involves prediction of post-operative surgical complications, long-term quality of life, and survival outcomes after surgery.

My thesis examines predictors of both short and long-term outcome in patients undergoing pancreatic resection for pancreatic cancer. The accurate prediction of short-term outcomes would allow the implementation of early recovery protocols in low risk patients, and vigilance leading to early identification of impending complications in high risk patients. The accurate prediction of long-term outcomes would allow identification of patients who would not oncologically benefit from resection, saving them the risks of surgery, allowing them to be directed instead towards systemic therapy.
