Associate Professor Dr Paul Robinson is a clinician researcher within the University of Sydney’s Discipline of Paediatrics and Child Health and is a respiratory paediatrician at the Children’s Hospital at Westmead. He is an established member of The Children’s Chest Research Centre, within the Kids Research Institute.

His work aims targets early detection and improved monitoring of paediatric lung disease through the development and translation into clinical practice of novel peripheral airway function tests. These tests are highly feasible throughout the paediatric age range (including infants and pre-schoolers). These “peripheral airways” may be small in size but account for a vast amount of the total lung volume (almost 95%). Conventional lung function techniques fail to assess these smaller airways, leading many to previously view them as existing within a “silent lung zone” where disease could develop undetected. Its importance in disease emergence and progression has already been confirmed in important diseases, such as cystic fibrosis, asthma, and COPD and is being established in other settings, such as post lung and/or bone marrow transplantation.

Dr Robinson’s research focuses on two particular peripheral airway function techniques, Multiple breath washout (MBW) and the Forced Oscillation Technique (FOT). He leads international MBW standardisation work to ensure testing is not only performed correctly, but efficiently and with equipment that accurately reflects what is happening in the lung periphery. Ongoing local research is funded by both national and international grants through strong collaborative networks established with leading researchers. “Having managed these children for many years, and seen how established lung disease can be before our previous techniques detected impairment, this work is critical to improve long term outcomes by detecting and intervening at an earlier stage” he says “In established disease, these tests also offer exciting potential to achieve better disease stability by targeting this lung region”.

Research interests

Conventional lung function tests (such as spirometry) are insensitive to changes occurring in the lung periphery. Imaging studies have shown us that by the time these tests become abnormal, lung disease is not only established but may be extensive. Novel lung function tests, targeting the peripheral airways, can assess this important lung region and importantly are also feasible in children of all ages. The long-term objective of Dr Paul Robinson’s research is to translate these promising tests into useful clinical tools for both children and adults. This will facilitate not only better understanding of how and when lung disease develops but also the detection of initial impairment so that earlier intervention can lead to better short- and long-term outcomes for patients. He continues to lead international standardisation work for inert gas washout, not only for adults, adolescents and school-aged children, but also more recently for preschool children (defined as 2-6 years of age). International consensus and technical standards documents have led to the greater availability of robust commercial equipment for both researchers and clinicians as interest in these techniques grows rapidly.

Building on the successful outcomes of this work, he is now focusing on defining clinical utility by using these tools (MBW, FOT) in conjunction with advanced imaging techniques, where applicable, in a range of important disease groups. Over half of preschoolers with Cystic Fibrosis will have abnormalities on MBW if used, but routine screening is yet to be adopted internationally. Disease processes evolving in the peripheral airways are major causes of morbidity and mortality following both lung and bone marrow transplantation, and are poorly responsive to treatment once detectable using spirometry. Asthma, which affects 10% of children and is an ongoing source of morbidity and mortality and indicates that the current symptom-based approach to asthma control is insufficient. Collaborative projects currently focus on (1) defining the optimal approach to early lung disease detection in CF and following bone marrow transplantation (2) better understanding the impact of maternal asthma control during pregnancy on the infants lung and immune system development and later asthma risk, and (3) establishing home-based FOT testing in older children to see if day-to-day monitoring can improve asthma control and facilitate earlier detection of exacerbations.

Teaching and supervision

As part of his role, he is involved with the teaching of medical students at various stages of their medical degree, the ongoing training of junior medical staff and respiratory and sleep trainees, and the supervision of higher research degrees, including current PhD candidates.
