Learn Now; Live Well: An educational program for patients and caregivers living with life-threatening illness
The Learn Now, Live Well program was designed to prepare informal care givers with relevant education and resources to care for someone with a life limiting illness at home. The Learn Now; Live Well project aimed to improve the confidence and knowledge to the carers of people living with life threatening illness through the provision of education and support.
The LNLW program consists of six modules covering topics such as pain, medications, symptom control, nutrition & exercise, communication and the physical side of caring. The program was delivered through Hollywood Private Hospital and through the community settings successfully. In March 2006 further funding was obtained to develop the program into a Train the Trainer package.
