Report a hazard, incident or injury

All hazards, incidents and injuries must be reported to the relevant University manager or supervisor as soon as reasonably possible.

Managers and supervisors are required to use RiskWare to formally record hazards, incidents and injuries relating to University activities. Records can be created on behalf of others, including students, contractors or visitors.

All incident's must be formally recorded within 24 hours of the incident occuring.

What is an incident?

An incident is a discrete (one off) occurrence that has, or could have had, an adverse impact on people. This can include: incidents resulting in injury, illness, equipment failure, or “near misses” when there is potential for an injury.

The incident report serves as a record for future reference should there be any subsequent complication e.g. delayed health effects, that may require the report as a factual base. Read more about the regulatory framework.

What is a hazard?

A hazard may be anything that has the potential to cause injury or illness to people, or damage to plant or equipment.

The reporting of incidents and identification of hazards is critical in achieving a safe workplace as it prompts action to prevent any future accidents and injuries.

Reporting an injury

The details of any work related injury or illness must be recorded at the time of submitting an incident report via RiskWare. If this cannot be done within 24 hours of occurrence please contact the Injury Management team for immediate assistance.
